Instructions for Diabetic/Orthopedic Shoes


General Description:

The shoes/orthotics (inserts) you have been fitted with are prescribed and designed specifically for you and to suit your unique foot type. Orthotics (inserts) can improve foot function and reduce or relieve your pain symptoms in your feet and legs by reducing abnormal structural and soft tissue stress. To do this requires the shoes/orthotics to redistribute pressures of forces. Therefore, a gradual break-in period is recommended in order to reduce or eliminate any potential discomfort. Please keep in mind that this is only a guideline and that your wearing protocol may be specific depending on your diagnosis.

Application:

Stockings: Make sure to have proper stockings, wear clean cotton or diabetic stockings. Make sure to smooth out all wrinkles, which can cause irritation or breakdown over time. Shoes: Use the shoe horn every time you put on your shoes. This will prevent the heel counters from breaking down. Use a good wax polish as needed to keep the leather soft and looking good. Allow the leather to air dry.

Laces: Do not overtighten your laces. Remember that your sensation may be compromised and overtightening you shoes can cause problems with circulation and skin breakdown. Make your shoes snug, but never tight. Keep your shoe laced tight enough so your feet will not slide forward in the shoes. Do not lace your shoes so tightly the laces cause pressure marks.

Schedule:

Day 1- Wear shoes/orthotics for up to one hour in the morning, if comfortable. Remove your shoes and socks and check for any skin irritation or pressure points. Leave the shoes/orthotics off for up to two or three hours. Wear again for up to one hour in the afternoon, if comfortable. Remove shoes and socks and again check for skin irritation or pressure points.

Day 2- Wear shoes/orthotics for up to two hours in the morning, if comfortable. Remove shoes and socks and check for any skin irritations or pressure points. Leave the shoes/orthotics off for up to two to three hours. Wear again for up to two hours in the afternoon, if comfortable. Remove shoes and socks and check for skin irritation or pressure points.

Day 3-Wear shoes/orthotics for up to three hours in the morning, if comfortable. Remove shoes and socks and check for any skin irritations or pressure points. Leave the shoes/orthotics off for up to two to three hours. Wear again for up to three hours in the afternoon, if comfortable. Remove shoes and socks and check for skin irritation or pressure points.

Day 4-If the shoes/orthotics have been comfortable and no skin issues have been present during the initial three days, then the wearing time can be increased at your discretion. However, this may be further specified depending upon your diagnosis.

Reduce your wearing time or have a rest day if you are feeling tired or fatigued by the orthotics. Everyone responds differently to orthotics and this is only a guide.

Red Marks:

Daily inspections must be performed to observe all boney prominences and pressure areas to ensure there are no problems occurring from your new footwear and/or orthotics. It is common for red marks to appear after wearing your shoes. These red marks should disappear within 30 minutes. If the red marks do not disappear within 30 minutes, discontinue wearing the shoes and contact our office at 806-358-2343. Deep red or bruise like areas, blisters, lesions, ulcerations, excessive callused areas or bloody spots in shoes or socks should be reported immediately to our office or your referring physician. If you find an area of concern, discontinue use of the footwear immediately and contact our office.

Maintenance:

Do not attempt to modify or adjust the shoes/inserts on your own. Inspect them daily for cracking, loose or worn parts. Contact our office for an appointment for any modifications or concerns. It is your responsibility to contract our office for adjustments or repairs.
You may wipe down the shoes and inserts with a damp cloth to clean them. We recommend HAND WASHING only using a mild soap and warm water. The shoes and inserts should be completely dry before using them, allow them to air dry.
Changes in weight, activity, overall health or growth may cause your shoes or inserts to fit unsatisfactory. Visit with your physician or orthotist on a regular basis to see if any changes are necessary.

Physician Follow-Up:

You should be following up with your physician routinely after your devices are delivered. This is to give the physician an opportunity to review the devices and suggest any changes. We do not schedule your follow up appointments with your physician.
